What is home health care?
Home health care provides skilled medical services in the comfort of your home under a physician’s order. Services may include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social services, and home health aide support.
Care is delivered based on medical necessity and an individualized plan of care.
Who qualifies for home health services?
You may qualify if:
- You are under the care of a physician
• You require skilled nursing or therapy services
• You meet medical necessity criteria
• You are considered homebound under Medicare guidelines (if using Medicare)
Our intake team will verify eligibility and guide you through the process.
What does “homebound” mean?
Homebound generally means that leaving home requires considerable effort due to illness or injury. Patients may still leave home for medical appointments, religious services, or limited activities.
We can help determine if you meet this requirement.
Is home health covered by Medicare or insurance?
Home health services are typically covered by Medicare and many private insurance plans when eligibility criteria are met.
We will:
- Verify your insurance
• Confirm benefits
• Explain any potential out-of-pocket costs
• Handle billing directly with your insurer
How soon can services start?
Once we receive a physician order and verify eligibility, services typically begin within 24–48 hours, depending on clinical urgency.
For hospital discharges, we coordinate closely to ensure a smooth transition home.
How often will the nurse or therapist visit?
Visit frequency is determined by your physician-ordered plan of care and your medical needs.
Some patients may receive multiple visits per week, while others may require less frequent monitoring.
Your care plan is regularly reassessed and adjusted as needed.
What happens in an emergency?
If you experience a life-threatening emergency, chest pain, difficulty breathing, or severe symptoms — call 911 immediately.
For urgent but non-life-threatening concerns, contact our office so a nurse can provide guidance.

Can Pearlstone manage complex medical conditions at home?
Yes.
Our skilled nursing team is trained to manage:
- Wound care, including wound vac therapy
• Surgical recovery
• IV therapy (as ordered)
• Drain and tube management (including nephrostomy tubes and JP drains)
• Catheter care
• Respiratory monitoring
• Chronic disease management
All services are provided safely under physician supervision.
What is the role of the Home Health Aide?
Home Health Aides assist with daily living tasks such as bathing, hygiene, dressing, and mobility.
They work under the supervision of a Registered Nurse and are part of the overall care team.
Home Health Aide services are provided as part of skilled home health — not as standalone personal care.
Will I have the same nurse or therapist each visit?
We strive to provide continuity of care whenever possible. While scheduling may vary, our team communicates closely to ensure consistent and coordinated treatment.
How long will I receive home health services?
Services continue as long as skilled care is medically necessary and progress toward goals is being made.
When goals are achieved or skilled care is no longer required, we will coordinate a safe discharge plan.
Can I choose my home health provider?
Yes. Patients have the right to choose their home health agency.
If you or your family prefer Pearlstone, inform your physician or discharge planner.
What if I have concerns or want to file a complaint?
You have the right to voice concerns without fear of retaliation.
We encourage open communication and will promptly address any issue. Information on how to file a grievance is included in your Client Handbook.
How does Pearlstone coordinate with my doctor?
We maintain active communication with your physician by:
- Reporting changes in condition
• Updating plans of care
• Reviewing medications
• Coordinating treatment adjustments
Your doctor remains in charge of your care.
Do I need advance directives or a POLST form?
While not required, we encourage advance care planning.
Documents such as:
- Advance Directives
• Healthcare Power of Attorney
• POLST (when applicable)
Help ensure your wishes are honored.
Our social workers can assist with discussions and guidance.
How do I start services?
Getting started is simple:
- Contact our office or submit a referral.
- We verify eligibility and obtain physician orders.
- An initial assessment is scheduled.
- Your care plan begins.
